Required Documents

Please have the following documents ready to submit with your application:

Identification
  • Valid photo ID (driver's license, state ID, passport)
  • Social Security card or number
  • Birth certificates for all household members
Income Verification
  • Last 3 months of pay stubs
  • Benefit award letters (SSI, SSDI, TANF, etc.)
  • Most recent tax return
  • Child support documentation (if applicable)
Additional Documents
  • Proof of Philadelphia residency (utility bill, lease)
  • Medical documentation (for healthcare program)
  • Eviction notice or proof of housing instability (if applicable)
  • Proof of appointment/work schedule (for transportation program)

How long does the application process take?

The application review process typically takes 5-7 business days. After your application is reviewed, we'll schedule an interview within 1-2 weeks. The entire process from application to program enrollment usually takes 2-4 weeks, depending on the program and your specific situation.

Can I apply for multiple programs?

Yes, you can apply for multiple programs if you need various types of assistance. Many of our clients benefit from a combination of our services. You'll need to complete a separate application for each program, but you can use the same documentation for all applications.

What if I don't have all the required documents?

We understand that gathering all documents can be challenging. Please submit your application with whatever documents you have available. Our team will work with you to identify alternative documentation or help you obtain the necessary paperwork.

Is there a waiting list for services?

Depending on current demand and funding, some programs may have a waiting list. Priority is given to those with the most urgent needs. Even if there is a waiting list, we encourage you to apply as soon as possible to secure your place in line and we can often provide interim support services.

What happens after I submit my application?

After submission, you'll receive a confirmation email or call within 1-2 business days. Our team will review your application and contact you to schedule an interview. During the interview, we'll discuss your needs in detail and explain how our programs can help you.

Is my information kept confidential?

Yes, all information provided in your application is kept strictly confidential. We adhere to all privacy laws and regulations. Your information will only be used to determine eligibility and provide appropriate services. We will never share your information without your explicit consent.
